IMPORTANT INSTRUCTIONS:
- Click on the ONLINE ENROLLMENT button below.
- Fill out the form with your name, email address, and YOUR CLIENT’S legal name and email address.
- Then click the BEGIN SIGNING button at the bottom of the form.
- Go to your email inbox to retrieve your signing validation code, copy it, and paste it to access the enrollment application.
- Your client’s name, email address, and the EFFECTIVE DATE will automatically fill in for you after completing the enrollment process.
- Ask your client to find the email from DocuSign in their email inbox and click the REVIEW DOCUMENT button.
- Your client must answer the 3 Suitability questions and click to sign the application, terms & conditions, and the Statement of Understanding pages.
- Click the FINISH button.
- You, your client, and True Freedom will receive a new email from DocuSign with the FIELD ISSUED CONTRACT for record keeping.
